Home Based Support

HOME BASED SUPPORT is a program designed to provide an array of quality, outcome-based, person- and community-centered services and supports for individuals with developmental disabilities and their families in Illinois up to a maximum monthly cost.

At the request of families, The Arc Home Based Support Program provides support that is adequate to assure the general health and safety of waiver participants, promote healthy lifestyles in the community, provide supports in order for the individual to live as independently as possible in the community setting of their choice and improve the individual’s access to multiple community environments.

The Home Based Support Program, under the Medicaid waiver program, is grounded in the following principles:

Person-centered supports – goals individual has identified as important to the individual

Individual-driven supports – informed individuals can direct supports they receive

3. Shared responsibility and risk – individuals assume relevant responsibilities and risk

4. Cost-effectiveness – waiver funds will be used adequately and appropriately for services

5. Benefit to the individual – supports will be of clear benefit to the individual

6. Appropriate use of public funds – used only to secure supports defined by service plan

7. No duplication – supports purchased will not be duplicative of each other

8. Payor of last resort – funds used only after funds from Medicaid State Plan and other resources for which individual is eligible have been maximized

9. Qualified providers – all providers are adequately qualified to provide support to the individual

     

The Home Based Support Program

allocates up to: $1869 per month per adult over 21

or $1246 per month for individuals

still in school, aged 18 to 21,

for  services such as: case management,

day training program, 

personal support,  mileage

and other services described  in

the DHS Home Based Manual

ELIGIBILITY This program is available to individuals who meet: THE ILLINOIS DEPARTMENT OF HUMAN SERVICES’ DEFINITION OF A DEVELOPMENTAL DISABILITY:
